YOUR RIGHTS AND RESPONSIBILITIES

PATIENT’S RIGHTS

  • Be treated relatively with access to medical treatment and others without regard to race, gender, creed, religion, and origin.
  • Be permitted to carryout his or her rights without being the subject of discrimination or reprisal.
  • Be the first to be informed concerning diagnosis, right treatment, procedure/s and prognosis, and the potential risks and effects in relation to both treatment/s and procedure/s.
  • Express dissatisfaction in relation to the treatment/s or procedure/s.
  • Receive a safe and confidential treatment environment in which patients are treated with consideration, respect, and dignity.
  • Be free from all sorts of abuse, and or harassment;
  • Be fully informed of the services offered at the Facility of Center.
  • Privacy of all information and records about the patient’s treatment;
  • Receive a notification of any breach of protected health information.
  • Be informed on the treatment or procedure before it is performed;
  • Receive necessary information from his/her physician as a consent before starting any non-emergency treatment.
  • Refuse any treatment as permitted by law and be fully informed of the consequences of that refusal. As such, it is the patient’s responsibility the moment he or she refuses the treatment or for not following the instructions directed by the physician or the facility.
  • Patients are provided, as appropriate, with information concerning their diagnosis, evaluation, treatment, and prognosis. If it is medically not recommended to give such information to a patient, the information can be disclosed to another person designated by that patient or to a legally authorized family member or guardian of the patient.
  • Receive access or copies of his or her medical records;
  • Be fully informed of the facility’s policy in regards to advance directives;
  • Receive and provide information regarding the DNR.
  • Prior to the transfer to another facility or organization, ensure that the receiving facility has accepted the transfer of the patient and has the appropriate medical records and equipment available.
  • Refuse to engage in any experimental research.

  • The facility agrees to comply with any applicable Federal Civil Rights Laws which assure equal access and service to individuals who are not proficient in English.
  • Expect to receive pain management services.
  • Expect that the facility staff members are covered under professional liability policy;
  • Be informed of the provisions for after-hours and emergency care during your stay at our facility.
  • Be fully informed of the service charges, or any eligibility for the third-party reimbursements when applicable.
  • Expect to receive an itemized copy of the account statement, upon request.

PATIENT'S RESPONSIBILITIES

  • Be sensitive to other patients and personnel by not calling attention to yourself, eating and other distractions.
  • Be considerate of Center’s Rules and Regulations
  • Be respectful of others’ property.
  • Communicate if he or she understands the treatment well, including what is expected from him or her.
  • Ensure to attend to appointments as scheduled, or if unable to do so, notify the facility and physician.
  • Provide full content to his/her procedures
  • Inform physician with his/her concerns and questions
  • Obey his/her treatment plan provided by provider/physician
  • Informing us if you do not understand any procedures or instructions by the Center or Facility.
  • Completing information accurately to Center’s staff and caregivers regarding medications, past illnesses, changes in patient’s condition or other health matters.
  • Understanding the rules of the facility during his or her stay, as well as the treatment or procedure/s, and if instructions are not executed.
  • Carry out  his or her financial obligations to the Center in a timely manner.
  • Providing essential information regarding any third-party payment sources
  • Acknowledge responsibility for any charges not covered by financing.
